  • If you did not receive an email notice from the Settlement Administrator, you must provide proof of purchase such as a sales receipt, store record, or photo of the mattress label or tag displaying the SKU, item number, or serial number. If you do not have proof of purchase, you must provide details about your purchase including the brand name, approximate date of purchase, store of purchase, size of the mattress, and the model or item name or number. The Settlement Administrator has the right to verify whether your claim is valid.

Siena has just one affected product, the Siena Memory Foam Mattress. DreamCloud has 3 affected SKUs covering the Premier and Premier Hybrid mattresses. Ashley has the largest list with roughly 120 affected item numbers.
